Patient Centered Medical Home

In April of 2011, East Granby Family Practice, LLC became one of the first practices in Connecticut to be certified as a Patient Centered Medical Home - Level 3 (highest level achieveable). East Granby Family Practice has always ascribed to the tenets of a Patient Centered Medical Home. East Granby Family Practice, LLC (EGFP) has been a Patient Centered Medical Home since April 2011. A Patient Centered Medical Home is a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange, and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner. As such, EGFP utilizes the tenants of the NCQA Medical Home process to provide the highest level of medical care to our patient population.

 

The National Committee for Quality Assurance (NCQA) is a private, 501(c)(3) not-for-profit organization dedicated to improving health care quality. Since its founding in 1990, NCQA has been a central figure in driving improvement throughout the health care system, helping to elevate the issue of health care quality to the top of the national agenda. NCQA makes this process possible in health care by developing quality standards and performance measures for a broad range of health care entities. These measures and standards are the tools that organizations and individuals can use to identify opportunities for improvement. The annual reporting of performance against such measures has become a focal point for the media, consumers, and health plans, which use these results to set their improvement agendas for the following year.

 

East Granby Family Practice has for many years ascribed to many of the tenets of a Patient Centered Medical Home, even before the formal NCQA PCMH recognition program was formulated. As Family Medicine Board certified physicians, the goals of East Granby Family Practice have always been to provide cost effective patient centered care. We have always had extended hours (evenings and weekends) and open access scheduling - especially since we have many pediatric patients who often need same day appointments. EGFP was one of the first medical practices in the state of Connecticut to adopt an EMR (electronic medical record) when EGFP purchased McKesson’s Practice Partner EHR in 2004. After 19 years that software was retired, and EGFP switched to Athena Health in September 2023. We strive for excellence in the care that we provide to our community, and over time have re-designed workflows and have established processes to involve all staff to work to their full level of training to deliver patient centered care.

About Us

 

EGFP, LLC is a full service medical office that provides both preventive care for children and adults, and medical treatment of pediatric and adult patients for chronic medical problems and acute illnesses. The providers perform office surgery, dermatological procedures, pulmonary function tests, radiological tests, electrocardiograms, immunizations, phlebotomy, audiology tests, and tympanography.  We service the communities north of Hartford, Connecticut and in south central Massachusetts.
