Appointments

Please try to anticipate your medical needs and request a mutually convenient time in advance.

 

East Granby Family Practice is currently putting a hold accepting new patients until later in 2023, and will be happy to see you then. We do not accept new patients with the need for chronic pain management. There are specialty practice that are better suited and able to help patients with this need.

 

When you make an appointment with us we are reserving time for your provider to spend with you. Whenever possible, we would like 24 hours notice if you are cancelling an appointment so that we can use that appointment time for another patient.  We make phone calls the previous day to remind patients of appointments, particularly if they were made in advance. There is a  $50 "No Show" charge if a physical, wellness exam or followup appointment is not kept.  The fee for a "No Show" for an acute visit is $25.

Insurance

We participate with most Health Insurance plans. To be sure we will accept your plan you can check with your carrier and verify we are participating providers or you may call the Billing Department at 860-653-0006.

We are happy to send your claims to your insurance carrier, but can only do so if we have the correct demographic and insurance information. This information is crucial to get your claims paid and avoid bills being sent to you. We will ask you if this information is correct at each visit, and if not, we will help you update it.

 

Insurance cards - We will check your insurance card at every visit to verify there are no changes to your policy and ensure your claim gets processed correctly. Please bring your insurance card with you to every visit and present it to the receptionist.

 

Co-payments - The majority of managed care companies have a patient co-payment, coinsurance, or deductible associated with most visits. When you contract with a managed care company, part of your agreement with them is to pay your co-pay at the time of service. We accept cash, checks and major credit cards.  There will be a $10.00 administration fee assessed if a co-pay is not paid at the time of service to defray the cost of billing you for these small amounts.

Deductibles - The amount you pay for covered services before your insurance plan will pay.  For example, the Medicare deductible for 2024 Medicare deductible is $240, so the patient would be responsible for this amount unless there is a secondary insurance with covers deductibles.  After you pay your deductible, you may still be responsible for a copayment or coinsurance.  If you would like to keep a credit card on file for paying these, please fill out this form to keep on file in our office.  For additional information, please visit HealthCare.gov or contact your insurance carrier.

Online bill pay - We now offer the ability to pay your balance online.  Please visit our "I Want To... Pay My Bill" page to access our online payment site, powered by InstaMed.

 

Referrals

Our referral expert will help you through this process.  It is your responsibility to contact our office for a referral if your managed care company requires one. When you see a provider here in the office and you are recommended to see a specialist this is not considered your "referral," but a recommendation.

 

In order to see a specialist, and have your insurance pay for that visit,  you must have authorization for the "referral."  Therefore, please set up the initial appointment with the specialist and then call our referral phone line at 860-653-0006. Give us the name of the provider you are seeing, the date of the appointment and the reason they are seeing you. We will then initialize the referral process, get authorization from your insurance company and send the approval to your specialist. If you forget to get your referral before the visit, most insurance companies will not allow the referral to be backdated.

Prescriptions

With our electronic medical records system prescription refills are done electronically between your pharmacy and your provider. When you are running low on your medication please contact your pharmacy to request a refill and they will send us an electronic message through a service called SureScripts. We request that you give us a 3 day advance notice. If you are completely out of medication please call the main office. Although we make a great effort to renew your prescriptions the same day, it may not always be possible as your provider may be out of the office. Please plan ahead and give us ample time to provide this service to you. Only emergency prescription medications are refilled over the weekend. We refill most medications directly through our electronic medical records to your pharmacy.  If you need your prescriptions written, we ask that you come in and pick them up to insure that you get them. No narcotics are filled without an appointment. Many prescriptions require a Prior Authorization from your insurance, see this message.

Health Forms

Health forms will be completed in a one-week time frame. If you have an appointment and need a form completed related to this appointment please bring the form in at that time. If forms are brought in without an appointment there is a $5.00 fee to be paid upon completion. We ask that when dropping the forms off that you complete as much of the form as possible. Once completed, you may pick them up, or we will mail them to you if you provide a self addressed stamped envelope.

About Us

 

EGFP, LLC is a full service medical office that provides both preventive care for children and adults, and medical treatment of pediatric and adult patients for chronic medical problems and acute illnesses. The providers perform office surgery, dermatological procedures, pulmonary function tests, radiological tests, electrocardiograms, immunizations, phlebotomy, audiology tests, and tympanography.  We service the communities north of Hartford, Connecticut and in south central Massachusetts.
